Yes. All tests on Equip are proctored. This article explains the different proctoring mechanisms in place. If you are creating a Custom Quiz, you can choose the proctoring settings that are enabled.
If you are creating a Custom Quiz, you can actually control how the questions are shown. So, the answer is both! In the former, you can set the time at the level of the entire quiz. With the latter, you can set time at the level of each question.
